Conduct a thorough patient profile review for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received; document relevant information in patient drug therapy.Twelve months of training in all aspects of advanced pharmacy practice: acute patient care, ambulatory patient care, drug information, drug formulary development, and practice management.Work collaboratively with decentralized clinical pharmacists to gain perspective of both acute patient care and ambulatory patient care. Gain experience in medication compounding and preparation of intravenous mixtures. Receive teaching on formulary management and policy development. Mentor by clinical pharmacist in the areas of my interest.
