Jesse J Farmer Email & Phone Number | •Responsible for conducting initial assessments and investigations for incoming adult protective referrals •Complete in depth sixty day assessments and investigations in order to determine validity of referrals and level of intervention needed by the department of social services •Maintain an active caseload of approximately 10-20 adult protective cases •Maintain extensive case notes and documentation for each active case •Conduct bi-weekly, face to face home visits with active clients •Open ongoing cases on WMS and ASAP programs when representative payee or guardianship services are necessary •Actively communicate with service providers, social service agencies, physicians, hospitals, and social workers in order to further investigate adult protective referrals. •Responsible for all management and operations of a 12 bed sober living facility for chemically dependent men •Manage all financials including rent collection, petty cash, payroll, expense reports, invoices, and monthly and annual budgets •Provide residents with case management services including referrals to local agencies and drug rehabilitation outpatient clinics, benefit applications, and one on one counseling •Responsible for the management, scheduling, and supervision of six employees •Provide residents with various housing and sober living resources upon discharge from the residence •Network with drug rehabilitation inpatient centers in order to gain referrals and build positive relationships within the community. • Provide social services and case management to individuals and families residing in permanent supportive housing with ailments such as mental illness, substance abuse, traumatic brain injuries, and developmental disabilities • Assist clients with obtaining state and federal benefits such as Medicaid, Medicare, and Social Security Disability • Create service plans for clients with the ultimate goal of promoting self-sufficiency • Coordinate and schedule monthly educational events with outside providers in order to assist clients with becoming more resourceful and independent • Provide weekly counseling sessions with clients covering topics such as drug and alcohol abuse, health, hygiene, depression and mental illness, legal services, and money management.
