Assist Program Director/ Principal Investigator and financial analyst with budget, purchasing and fund allocation for awarded grants. Assist Medical Director with grant application preparation, submission and tracking of grant cycles Screen and recruit subjects participating in PREP (Personal Responsibility Education Program) Promote enrollment and compliance with subjects participating in PREP Act as one of the educators for PREP classes to educate teen parents about positive relationships and healthy sexual practices Coordinate with providers to enhance referral process Purchase office and clinical supplies for South Main Public Health Center, Foster Care Program, and Teen Mother and Child Program Coordination of meetings with outside entities and community partners Co-facilitate group prenatal care Coordinate support staff scheduling Volunteer coordination Assist with implementation of new policies and procedures for clinic Attend health fairs to educate outside agencies and the community of TMCP services Super-User for EPIC for Business Develop and coordinate case management plans with teen parents Provide one-on-one educational counseling to teen mothers and fathers Assess client eligibility for resources and provide referrals to those agencies Meet with clients in a clinic setting, chart patient goals, progress and follow-up Set appointments for GED testing, Home & Hospital services, home study and night school Attend meetings with community agencies Update community resources, statistics and create new information for client’s use.
