Such experiences provided me the opportunity to work in various settings, such as health care and mental health. Some of my roles included counselor, advocate, and educator. In my current role as a hospital social worker, I coordinate discharge planning that is both effective and efficient for the patient and hospital. I rely upon my medical social work knowledge and skills to appropriately assess, direct and implement discharge plans that are timely, safe and address the patient’s current level of care. Additionally, I work daily in collaboration with other hospital disciplines (i.e. Physicians, Therapists and Nurses) to develop a cohesive and all-encompassing discharge plan for the patient. Furthermore, I maintain good working relationships and contacts with outside healthcare and community agencies (i.e. Skilled Nursing Facilities, Home Health Care providers, and Durable Medical Equipment companies) to ensure smooth transition of care from the hospital. As a result of these experiences and my current position, I am constantly cultivating my social work and case management knowledge.
